YCQ SCHOOL HEALTH REQUIREMENTS AND REQUIRED MEDICAL FORMS

As per the Department of Health (DOH), children attending day care and pre-K through 12thgrade in New York State must receive all required doses of vaccines on the recommended schedule in order to attend or remain in school. 

Students in UPK, Kindergarten, and Grades 1, 3, 5, 6, 7 are required to have a medical exam and completed form CH205 (See attached) with date of exam doctors signature and stamp/license no.

BEFORE SCHOOL BEGINS ONLY

To have your child(ren) cleared to begin school on-time

IF you are missing documents, we will notify you via your ParentLocker email.

IF you do not complete YCQ Electronic Health Form prior to the beginning of school your child will not be cleared.

Students with any of the following conditions are required to fill out the attached Medication Administration Forms (MAF)- PEASE FILL OUT ALL PAGES (Physician and Parent)

  • Asthma
  • Allergies
  • General Medication
  • Diabetes
  • Seizure Disorders

ONCE SCHOOL BEGINS

Email all documents to jjaffe@ycq.us with each child’s full name and class in the subject line.

ALL immunizations and Health Forms are required no later than 14 days from the first day of school.

***ALL STUDENTS ANTICIPATING TRYING OUT AND/OR PARTICIPATING ON A YCQ ATHLETIC TEAM MUST HAVE A PHYSICAL AND COMPLETED FORM ON FILE.

ALL students in grade 6 require a Tdap booster to begin school (Unless birthdays are SEPT.-DEC.- requiring by birthday)

ALL students in grade 7 require the Meningococcal vaccine to begin school (Unless birthdays are SEPT.-DEC.- requiring by birthday)

PLEASE NOTE: NYS Law

  • New York State removed religious exemptions from school immunization requirements in 2019. This applies to all public, private, and religious schools. 
  • Consequences: Children who do not meet the vaccination requirements will be sent home from school.

YCQ Immunization Policy

Yeshiva of Central Queens adheres to the NYS Law and immunization policy, procedures, and requirements of the Department of Health and Mental Hygiene (DOHMH).

Under New York law, all schools are prohibited from admitting any student without a certificate of immunization from a healthcare practitioner.  The immunizations required vary by age and grade. For all requirements consult the YCQ School Health page; your doctor; or Mrs. Jaffe. 

If you are transferring to YCQ from out-of-state or another country and are in the process of obtaining proof of immunization or a certificate of immunization, your child may attend for 30 days while providing the required documentation.

If you cannot afford the immunizations, your local health authority will administer or arrange for the administration of the immunizations.

If you believe you have a legitimate medical exemption under New York law, your application must be approved by the school prior to the expiration of the state-mandated time periods.  Your application may be rejected, in which case we will advise you of the reasons.

Medical exemptions or deferrals, if granted, must be reissued annually.  Without a certificate of immunization or an approved exemption, a child will not be permitted to attend classes.  

According to Public health law section 2164, there are no religious exemptions from required vaccines for school attendance.

N.Y. State Education Law requires a signed and dated certificate of immunization from a physician or health clinic to be on record for each entering student.  It is crucial that each student’s immunization record is uploaded and submitted BEFORE the start of the school year. Students without an immunization record on file will be unable to attend school in September.

Any child without documentation of the required immunizations will be excluded from school until such documentation is received.
